The Office of Student Administrative Services Financial Aid Compliance is committed to protecting your personal information, and we take reports of identity theft very seriously. If you believe someone has fraudulently used your identity to create a student account at 七色视频, submitting a claim by completing and returning an Affidavit to us is the first step.
Once we receive your Affidavit, we will conduct an investigation, which may include analysis of University records, IT data, interviews, etc. If we confirm that identity theft has taken place, we鈥檒l correct our records accordingly and take additional available steps, including updating your credit report where necessary. We will also provide you with instructions about steps that you should consider taking.
If you recently received a 1098-T tax form and you believe you may be the victim of identity theft, please see below for instructions on how to formally report identity theft with the University. Provided we substantiate identity theft during our investigation, we will work with the IRS to revise and refile a corrected 1098-T, as needed.听Importantly, please also note that the IRS issues . Please see 鈥淪teps for victims of identity theft鈥 and take appropriate action. In the meantime, the 1098-T can be ignored and need not be filed with your tax return.

You should anticipate up to 60 days from our receipt of a completed Affidavit to conclude our investigation.
