Identity theft reports

The Office of Student Administrative Services Financial Aid Compliance is committed to protecting your personal information, and we take reports of identity theft very seriously. If you believe someone has fraudulently used your identity to create a student account at 七色视频, submitting a claim by completing and returning an Affidavit to us is the first step.

Once we receive your Affidavit, we will conduct an investigation, which may include analysis of University records, IT data, interviews, etc. If we confirm that identity theft has taken place, we鈥檒l correct our records accordingly and take additional available steps, including updating your credit report where necessary. We will also provide you with instructions about steps that you should consider taking.

If you recently received a 1098-T tax form and you believe you may be the victim of identity theft, please see below for instructions on how to formally report identity theft with the University. Provided we substantiate identity theft during our investigation, we will work with the IRS to revise and refile a corrected 1098-T, as needed.听Importantly, please also note that the IRS issues . Please see 鈥淪teps for victims of identity theft鈥 and take appropriate action. In the meantime, the 1098-T can be ignored and need not be filed with your tax return.

Identity Theft Claim Process

To file a claim with the University, submit an ID Theft Affidavit packet. This consists of:

  1. A completed and听notarizedID Theft Affidavit听(PDF)
  2. A copy of an incident report of ID theft from your local police department
  3. A听clear copy听of a valid government-issued photo identification card (e.g. Driver鈥檚 license, passport, and/or state issued ID)

Please fax, email, or mail these documents using the information below. Email is generally the most expedient form of submission. As soon as we receive the required paperwork, an investigator will contact you to begin looking into your case. You should anticipate up to 60 days from our receipt of a completed Affidavit to conclude our investigation.

Frequently asked questions

  • All universities are required to send the 1098-T tax form to individuals with qualified educational expenses at the institution by January 31st of each year.
  • You received a 1098-T from 七色视频 because our business records show an account in your name, using your SSN and other Personal Identifying Information (PII).听 Because you are stating that you have not been a student at the University, you may be the victim of identity theft, and the account in your name may have been opened by a fraudster.听
  • We will immediately freeze the account that was opened in your name to ensure that no further action can be taken, pending our investigation.
  • We deeply regret this situation and any inconvenience this incident may cause you. The University maintains continuous and evolving mechanisms to prevent and detect identity theft. We investigate allegations of identity theft and report any instances that do occur to the Department of Education Office of Inspector General.听
  • We will do everything we can to help you resolve this matter, but please understand that our authority is limited.听 As the victim of identity theft, there actions that only you can take.听听

  • The University maintains continuous and evolving mechanisms to prevent identity theft and to investigate and report any instances that do occur. We will do everything we can to help you resolve this matter.听 To begin, we will freeze the account in your name at the University to ensure that no further activity can occur, pending our investigation of the matter.
  • To officially submit a report of identity theft to us, follow the Identity Theft Claims Process steps detailed above.
  • After completing the Affidavit, please return it along with the required supporting documentation per the instructions provided so we can investigate your issue related to the enrollment in your name at 七色视频. The completed Affidavit can be emailed to ID.theft@phoenix.edu, faxed to 602-626-1669, or mailed to SAS FA Compliance, 4035 S. Riverpoint Pkwy, Mail Stop CF-K608, Phoenix, AZ 85040.
  • Once we receive your complete ID Theft Affidavit, we will investigate your claim. That investigation could take up to 60 days to complete and we will notify you when complete. If the investigation substantiates the ID theft claim, we will take necessary corrective actions.

We are not experts on identity theft; however, according to , there are many ways your identity can be stolen, including in person, online, via social media, and by phone:

  • Large scale data breach
  • Stealing your wallet or purse to get ID, credit, or bank cards
  • Going through your trash to retrieve bank statements or tax documents
  • Installing skimmers at ATM machines, cash registers, and fuel pumps to digitally steal information from your bank card
  • Getting personal information from your phone when you use public Wi-Fi
  • Using 鈥減hishing鈥 to get information from you through fraudulent email, texts, or phone calls
  • Looking through your social media accounts to find identifying information in posts or photos. Or they may ask you for personal information in online quizzes and surveys.

  • Based on your initial report, we have frozen the account in your name at the University to ensure that no further action can be taken on it, pending our investigation.
  • The University maintains rigorous protections to prevent data breaches of PII in the University鈥檚 possession.听 We take identity theft and fraud very seriously.听 We maintain continuous and evolving tools and mechanisms to prevent and detect suspicious activity.
  • We work closely with the US Department of Education鈥檚 Office of the Inspector General to report suspected fraudulent behavior and work in concert with them and law enforcement to address identity theft when it occurs.
  • That said, while the University鈥檚 tools are effective, unfortunately we cannot prevent all instances of identity theft or student loan fraud.
  • Individuals need to protect themselves by utilizing tools and behaviors such as those described above.
